Procedures for Slide Duplication Requests: (last updated September 23, 2004)

  1. Visit PHIVOLCS Main Office (address written below this page) and bring with you 
        an unexposed slide film (Kodak Ektachrome, Fujichrome, etc.)
  2. Go to Geologic Disaster Awareness and PreparednessDivision(GDAPD,located at the 2nd flr.) 
  3. A GDAPD personnel will assist you  in selecting slides from the archives.
  4. Fill-up the request form, list down your selected slides in the space provided in the request form. 
  5. After the selection, the GDAPD personnel shall take the unexposed slide film 
        and a stub for service payment shall be given to the requisitioner.
  6. Pay for the amount indicated at the Cashier (1st floor).Duplication is 5 pesos per slide.
  7. The exposed slide film can be claimed after one(1) week because duplication is 
        done in bulk and depended on the bright morning sunlight. 
  8. Developing and processing of the exposed slide film is the responsibility of the owner. 
  9. Borrowing of the original slides are not allowed.
