Procedures for Video Dubbing Requests: (last update: September 23,2004)

  1. Visit PHIVOLCS Main Office (address written below this page) and bring with 
        you a video tape in VHS format. 
  2. Go to GDAP Division (located at the 2nd flr.) 
  3. A GDAPD personnel will assist you  in selecting documentaries from the collection.
  4. Fill-up the request form. List down your selected documentaries in the space provided in the request form. A maximum of 2 documentaries is allowed per requisitioner. 
  5. After the selection, the GDAPD personnel shall take the VHS tape and a
        stub for service payment shall be given to the requisitioner.
  6. Pay for the amount indicated at the Cashier (first floor). Duplication is 5 pesos per documentary. 
  7. The tape can be claimed after one(1) week because duplication is done in bulk.
  8. Borrowing of the original tapes are not allowed.
