Document(s) required:
1) Clear copies of the TCT or tax declaration of the lot or lots to be assessed
2) Vicinity Map of lot or area to be certified showing easily-identifiable landmarks and in appropriate scale (preferably a google earth satellite image with the lot boundaries indicated).

* All documents submitted to PHIVOLCS shall be part of PHIVOLCS records. These will not be returned to the customer.

To request:
1. Submit request and upload all required documents at our Online Hazard Assessment Service (OHAS) Portal https://has.phivolcs.dost.gov.ph
2. Follow the instructions on the landing page for other necessary steps.
3. Claim your Signed Hazard Assessment Report at the OHAS Portal. It is important to note your Tracking Number to track the status of your request and download the Signed Hazard Assessment Report.

Processing Time (Depending on complexity of request):
a. SIMPLE ~3 days
b. COMPLEX ~7 days
c. HIGHLY TECHNICAL ~up to 20 days

Fees:
Php 500 for brokers and developers (business use)
Php 100 for homeowners (for personal use)
Free for Government Agencies and Local Government Units
