REQUEST FOR QUOTATION
TO ALL INTERESTED SUPPLIERS:
The PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY (PHIVOLCS) hereby invites all interested suppliers to quote their lowest price on the item/s listed below, subject to the conditions stated below, stating the shortest time of delivery and submit sealed quotation to BAC Secretariat addressed to the BAC Chairman at PHIVOLCS located at PHIVOLCS Building, C.P. Garcia Avenue, U.P. Campus, Diliman, Q.C. not later November 13, 2018. The Approved Budget for the Contract is P 234,000.00.
One Hundred Twenty (120) sets REDAS Convention Meals (November 27-29, 2018) at PHIVOLCS Auditorium
- Lunch, Morning and Afternoon snacks with the following meal requirements:
Buffet lunch to consist of soup with Main entrée (vegetable, fish (no fish fillet), beef/pork/chicken, fresh fruit dessert, rice, bottled juice, free flowing coffee, fresh milk, tea, 10 servings of chips/nuts/candies per day
Menu to be pre-approved by requestee. No MSG, Magic Sarap, and similar condiments in the meals
Subject to food test and inspect of kitchen/preparation facility for cleanliness
Kindly submit your best offer with the following requirements:
- Mayor’s/Business Permit
- PHILGEPS Registration Number
Conditions:
- All entries must be typewritten.
- Delivery period, ASAP.
- Price validity period must be for a period of one hundred (120) calendar days
