TO ALL INTERESTED SUPPLIERS:
The Philippine Institute of Volcanology and Seismology (PHIVOLCS) hereby invites all interested suppliers to quote their lowest price on the item/s listed below, subject to the conditions stated below, stating the shortest time of delivery and submit sealed quotation to BAC Secretariat addressed to the BAC Chairman at PHIVOLCS located at C.P. Garcia Avenue, UP Campus, Diliman, Quezon City not later than 5:00 P.M. of February 28, 2018. The ABC is Php 55,000.00:
- One (1) lot Van Rental
- 10-15 Seater Van or Mid-to-Full-Size SUV and
Date of Travel and Destination:
- March 17 – 21, 2019 (5 days) – Abra province and vicinity and vice versa (10-15 Seater Van or Mid-to-Full-Size SUV)
- March 22 – 25, 2019 (5 days) – La Union province and vicinity and vice versa (10-15 Seater Van or Mid-to-Full-Size SUV)
- March 26 – April 2, 2019 (8 days) – Benguet Province and vicinity and vice versa (10-15 Seater Van or Mid-to-Full-Size SUV)
- Number of Personnel: minimum of 5 Dynaslope Personnel with equipment
- Departure Time from PHIVOLCS, Quezon City: preferably 5:00 AM
- Departure Time from Destinations: preferably 5:00 PM
- Billing Statement is to be issued by the supplier for every trip ticket for payment processing
- Net cost inclusive of van rentals, driver’s food and accommodation, emergency vehicle repairs and maintenance, fines, from traffic violations, and all taxes.
- Exclusive of gas and toll
- Travel may be cut short during the trip by at most 50% of the total duration, without prior notice.
