TO ALL INTERESTED SUPPLIERS:
The PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY (PHIVOLCS) hereby invites all interested suppliers to quote their lowest price on the item/s listed below, subject to the conditions stated below, stating the shortest time of delivery and submit sealed quotation to BAC Secretariat addressed to the BAC Chairman at PHIVOLCS located at PHIVOLCS Building, C.P. Garcia Avenue, U.P. Campus, Diliman, Q.C. not later May 7, 2021. The Approved Budget for the Contract is ₽414,000.00.
One (1) lot Vehicle Rental in Butuan City and Vicinity
- May 17, 2021 to June 8, 2021 – Butuan City and Vicinity, Agusan Del Norte
- Three (3) Vehicle Rental with driver (gas included)
- Vehicles are good for Thirteen (13) hours
- Sports Utility Vehicle (SUV)
- Drivers must bring their own lunch meal during the service
- The quoted amount is tax inclusive
- Accommodation of drivers are not included nor provided
- Vehicle rentals service will be for weekdays only (Monday – Friday)
- For a total of seventeen (17) days only of rental service from May 17 to June 8, 2021
ITINERARY:
- May 17, 2021 – Travel from Butuan Airport to accommodation
- May 18, 2021 – Travel from accommodation to Butuan City Hall and city’s vicinity
- May 19, 2021 to June 7, 2021 – Butuan City and its vicinity
- June 8, 2021 – Travel from accommodation to Butuan Airport
- End Travel
Note:
Per IATF Travel Protocol, drivers must secure the following requirements (Barangay Health Certificate, Medical Travel Clearance Certification, Company Travel Order)
Travel schedules and duration may be subject for changes depending on any delays that are related to the processing of travel requirements and other LGU health protocols

- PHILGEPS Registration Number
Conditions:
- All entries must be typewritten.
- Delivery period, ASAP.
- Price validity period must be for a period of one hundred (120) calendar days
