REQUEST FOR QUOTATION
TO ALL INTERESTED SUPPLIERS:
The PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY (PHIVOLCS) hereby invites all interested suppliers to quote their lowest price on the item/s listed below, subject to the conditions stated below, stating the shortest time of delivery and submit quotation to BAC Secretariat addressed to the BAC Chairman at PHIVOLCS located at PHIVOLCS Building, C.P. Garcia Avenue, U.P. Campus, Diliman, Q.C. not later March 22, 2021. The Approved Budget for the Contract is ₽65,000.00.
VEHICLE RENTAL
- 4×4 Vehicle or Pick-up truck
- Travel date: April 7 – 15, 2021
- Date of Travel and Destination
April 7 – 10 (4 days) – Bakun, Benguet and vicinity – 4×4 vehicle or pick-up truck
April 11 – 12 (2 days) – La Trinidad, Benguet and vicinity – 4×4 vehicle or pick-up truck
April 13 (1 day) – Bokod, Benguet and vicinity – 4×4 vehicle or pick-up truck
April 14 – 15 (2 days) – Tublay, Benguet and vicinity – 4×4 vehicle or pick-up truck
- Number of Personnel: minimum of 5 DYNASLOPE personnel with equipment
- Departure Time from PHIVOLCS, Quezon City: preferably 5:00 AM
- Departure Time from Destinations: preferably 5:00 PM
- A Billing Statement is to be issued by the supplier for every trip ticket for payment processing
- Net cost inclusive of van rentals, driver’s food and accommodation, emergency vehicle repairs and maintenance, fines, from traffic violations, and all taxes
- Inclusive of toll fees
- Exclusive of gas
- Travel may be cut short during the trip by at most 50% of the total duration, without prior notice
