REQUEST FOR QUOTATION
TO ALL INTERESTED SUPPLIERS:
The PHILIPPINE INSTITUTE OF VOLCANOLOGY AND SEISMOLOGY (PHIVOLCS) hereby invites all interested suppliers to quote their lowest price on the item/s listed below, subject to the conditions stated below, stating the shortest time of delivery and submit sealed quotation to BAC Secretariat addressed to the BAC Chairman at PHIVOLCS located at PHIVOLCS Building, C.P. Garcia Avenue, U.P. Campus, Diliman, Q.C. not later October 8, 2020. The Approved Budget for the Contract is ₽370,500.00.
Three (3) units Vehicle Rental in Cauayan City, Isabela and Vicinity
- October 12, 2020 to October 30, 2020 – Cauayan City, Isabel 6,500 pesos per car per day
- Three (3) car rental with driver (gas included) Vehicles are good for 13 hours preferably Van/SUV
- Drivers must bring their lunch meal during the service. The quoted amount is tax inclusive
- Accommodations of driver is not included nor provided
ITINERARY
- October 12, 2020 – Pick up of passengers from PHIVOLCS Bldg. Quezon City to destination (Cauayan City, Isabela)
- October 13 – 29, 2020 – within the vicinity of Cauayan City, Isabela only
- October 30, 2020- Alloted for trip from Cauayan City, Isabela back to Quezon City
- End of Travel
Note:
Per IATF Travel Protocol, drivers must secure the following requirements
(Barangay Health Certificate, Medical Travel Clearance Certifications,
Company Travel Order)
Kindly submit your best offer with the following requirements:
- Mayor’s/Business Permit
- PHILGEPS Registration Number
Conditions:
- All entries must be typewritten.
- Delivery period, ASAP.
- Price validity period must be for a period of one hundred (120) calendar days
